Every Monday, Alder Bay's twelve care coordinators built the week's outreach list by hand. Discharge data lived in the EHR, lab results in a separate system, prescriptions in the pharmacy platform, and claims in a warehouse only the analytics team could query.

Each list took about three hours, and it was stale by Tuesday. Patients who needed a call slipped through when a result landed in the one system nobody had checked.
